Care by Design is an individualized, coordinated care program aimed at helping senior adults.
“We are thrilled to be receiving funding from Virginia G. Piper Charitable Trust,” said Tom Egan, CEO of the Foundation for Senior Living. “With the population aging and the rising costs of healthcare, our new Care by Design program is going to be a valuable service to seniors and their caregivers.”
Care by Design will promote coordination of care throughout Foundation programming and will eliminate the missed opportunities that are currently presented to the Foundation to assist families and caregivers.
It will adopt the principles of a CHT coordinate a continuum of care for Foundation clients, irrespective of their point of entry into the Foundation’s spectrum of services.
The Foundation offers myriad in-home and community-based services that can improve quality of life for aging adults, adults who have experienced a change in health status, and adults with disabilities – and as importantly, their caregivers.
The Care by Design Program has four major goals:
- Ensure that clients’ unmet needs are identified and met through Foundation services if available.
- Connect clients to other community service providers when the Foundation does not offer services to meet the client’s unmet needs.
- Serve underinsured and uninsured clients through the program’s revenue surplus.
- Increase revenue to achieve program financial self-sustainability;
Outcomes include improved health and well-being and fewer medical events that result in admissions to hospitals or skilled nursing facilities.
Long-term outcomes include improved quality of life, aging in place, and reduced strain on community resources.
Short-term, clients and their caregivers will experience fewer barriers to care and smoother introduction to new in-home and community-based services.
Long-term, clients’ unmet needs will be identified and fulfilled quickly and prior to any negative consequences. Caregivers will be relieved of the responsibility of arranging appointments and securing in-home services.
